It's endemic. To fix it we must change the culture.
Since we no longer can count on decency or civility, I suggest we count instead on putting the blame where it belongs: the sexualization of children (Hannah Montana, anybody?) and the attendant "can't let x do me wrong" culture.

I agree: misogynistic men are the ones to focus on
Misogyny is caused by...misogyny (and not "sexual rejection" as various news outlets would have you believe about these latest hate crime kill spree). Misogynistic men become this way because they see men behaving this way. We've all grown up with it. It's our cultural norm for men and boys to express violent bigotry towards women and girls. Slurs are just common profanity. Rape images (whether real or simulated) are something to be seek out, view repeatedly, and masturbate to. It's entertainment to kill prostitutes, the easiest of female prey, on video games. We have comedians like Chris Rock talk about how tracking down your ex-wife and killing her like O.J. did is "sick," but that he "understands." So bigotry is fine, but bigoted violence is just wrong. Right.
When it comes to misogyny, we, as a society, but especially men and boys, openly justify or even celebrate hate crimes. These hate crimes are seen as morally just, entertaining, trivial, or "natural."
Just look at how many on the supposed left mock "missing white women" stories. Immediately you notice that the framing doesn't ridicule the media, but the dead victims. It also fails to mention the role misogyny plays. These women and girls are overwhelmingly targeted due to gender for rape and murder. These are simply hate crimes. Media coverage of gay bashings almost universally focuses on white male victims and yet those white men aren't similarly mocked, let alone in an overtly bigoted way. Also, the accusation of racism is a red herring since basically all domestic news is white news and if the media showed nothing but black women and girls who were raped and killed, guess who the likely guilty parties would be? That's right: black men. How would media coverage of black man after black man taken to jail on these charges combat racism? Especially since the media doesn't treat misogynistic hate crimes as serious, but as goddamn entertainment stories. Ever wonder why they're so damn popular? Because we enjoy them. We especially like sexualizing them (Hence, the "pretty" coed victims and the need to be gratuitous in detailing her suffering during the rape and whether or not she was found scantily clad).
Whenever anyone focuses on what women and girls can do to mitigate misogynistic bigotry, I not only remind them that it's men and boys who are committing these hate crimes, including rape, but that even if women and girls were basically erased from society and under total control of men and boys (so as not to offend our precious "feelings") misogyny would not only still be rampant, but increase dramatically. See: Afghanistan!
Men and boys are the answer, because we, as a group, are the ones causing the problem. If any guys are reading, I recommend Jackson Katz.
misogyny isn't strictly a male phenomenon, though
and neither is murder borne of jealousy. Just last year 56-year-old Jessica Kalish
served as a gruesome case in point, murdered by her estranged spouse, 57-year-old Carol Anne Burger.
That account, by the way, agrees with many others but leaves out some salient facts. Kalish and Burger were married, but the relationship had already begun to sour before the wedding, which some in retrospect suggest the couple undertook more as a political statement than a relationship commitment. Because Florida does not recognize same-sex marriage, they were not able to get a divorce (and the attendant split/settlement of property) in Florida; and because they were Florida residents, they couldn't get a divorce in the state where they'd married, either. Kalish had found another companion after the breakup of the relationship; Burger had a confidante, a friend, who isn't mentioned as a paramour. But the signs were there: an estranged spouse entangled in financial difficulties and an irreparable joint-property arrangement who dwelt on unhealthy emotional issues and a self-imposed helplessness past reason into madness.
Of course men often commit horrific crimes against their wives and families. They do it out of who-knows-what kind of mental illness -- rage, frustration, fear, jealousy? Overwhelming shame after rejection? Boys kill girls too, and the motives are often as unclear as the killers' identities.
But what arises out of this is that we don't need to assume women are safe companions for each other, either, as Sandra Cantu's murder is a tragic reminder. Of course, the Sunday School teacher's not the only person to look carefully at whenever a girl is endangered.

Men commit vast majority of misogynistic hate crimes
We're talking about groups here, not outliers. It's as simple as that. No one is assuming women and girls are incapable of committing gender-based violence against either gender, whether female (Sandra Cantu case) or male (see: all those women teachers convicted of child rape charges against male students).
Misgoynistic bigotry is the driving force of (heterosexual) domestic violence, not the favorite catch-all excuse:"hurt feelings" ("Overwhelming shame after rejection?" Are you serious? Why is that boys are especially "ashamed" when a girl rejects them?) It's like saying: Yes, during Jim Crow many blacks were lynched and terrorized by whites, specifically white men, but not all of it was due to racism. No, some of it was just "genuine" fear or anxiety about crime. It's trying to muddy the waters.
And it's not like batterers hide or sanitize their misogyny either. Just because misogynistic slurs, rape jokes, and general violent anti-female hatred are common in society doesn't make it any less notable, it just shows how deeply ingrained bigotry against women and girls is, which goes to explain why misogynistic violence--in which gender plays the determining factor in selection--is at damn epidemic levels.
Women and girls suffer a range of hurt feelings as well and yet they don't systematically target victims on the basis of their (male) gender. Men and boys do and women and girls are overwhelmingly their victims in such crimes. That's the key here: gender was the determining factor in selecting these victims for torture and slaughter. Incorporating sexuality (rape) or "love" (domestic violence) doesn't cancel out the effect or justify their behavior.
Hell, females don't even commit generic violent crime anywhere near the level that males do. Why? Because we're raised differently than women and girls. And that's without looking at bigoted violence, whether misogynistic, racist, or homophobic, which is almost universally committed by males. Or to take another example: Why is it that white police officers are way more likely to kill undercover black or Latino officers than vice-versa (assuming there have even been any cases)? Racism. You can claim that these white officers were just scared or that even black officers kill undercover officers of color, but that doesn't get at the root, "Why?" Specifically, why were they scared enough to shoot, when in a similar situation with a white person they weren't?
With regards to homosexual domestic violence: yes, neither gays or lesbians are immune from misogyny (or homophobia for that matter) either. What I do know about batterers, regardless of orientation, is that when they kill, they kill to keep. It's a final act of control and contempt, not mere jealousy or frustration. It's usually why they end up killing themselves afterwards: they lost their scapegoat, their coping mechanism.
You're right about one thing: women/girls use different weapons
when they attack.
Pitch me some links to buttress your contentions, please. Otherwise I'm going to take it as read that you want all domestic violence to be considered a hate crime as well.
Which absolutely defeats the purpose of the "hate crime" designation through dilution.

Yes, let's not forget the people who run the media companies
That sexualized these children, many times with the willing collusion of the parents.
I recently got a chance to finally watch War Inc with John Cusack and Hilary Duff, and I know Duff's performance was roundly panned, but I honestly thought it was absolutely spot on for her character. Everything about Yonica's life was completely manufactured, and she had absolutely no concept of life outside of her controlled bubble. So it was completely understandable that the character was stilted and awkward.

and of course Britney's parents made a ton of money off her early career. Don't know whether the Aguilera girl's parents were cajoled or not. Selena's family doesn't just miss her company, they miss her income. I've been reading some other sources -- there's a free / online paper out of Miami that did a long investigative piece on the HuffPo writer covering Obama's early campaign who, when her marriage broke up but she couldn't get a divorce, killed her wife -- with 222 stab wounds. Shortly after the body was found, she put a gun to her chin and blew the top of her own head off.
Can't blame all the violence on men. Can't blame all the rage on missed signals. But in that case ... well, in that case you'd have to say there was more than a touch of mental illness involved, and it revolved around sexuality in a way we're not supposed to mention. Jealousy is a wicked, wicked witch.
